These are pretty hard to find. No damage other than the box has a few issues.

It doesn't light up, so I'd guess the bulb is no good. Not much to go wrong other than that really. We don't have any bulbs like this to swap out. I think it's one of those old 7 watt Christmas tree bulbs.
